以白玉精心琢磨而成，玉质润泽，触手细腻，造型规整，端庄古雅。牌面浅浮雕有瑞兽纹及文字，牌首双龙，中部穿孔。正面开光内饰瑞兽纹样，瑞兽作蹲姿，首似鹿，角似羊，身披鬃鬣，口吐仙气，神情肃穆。背面开框刻“受天百禄”四字。“受天百禄”语出《诗经•小雅》：“天保定尔，俾尔戬谷。罄无不宜，受天百禄。降尔遐福，维日不足。”意为上天庇佑，可享福禄太平，受上天恩赐，万事皆宜。整件作品繁简得当，适度留白，将瑞兽之静与仙气之动完美结合，给人以庄重但不失灵动之感，且雕琢精美，寓意吉祥，或为清代玉牌盛行时期之佳作。
